Job summary

Clayton County, Georgia is seeking a Part-Time Case Manager for the Aging Program. The role provides case management and service coordination for older adults, assessing needs and coordinating home-delivered meals, in-home services, transportation, and community resources.

Responsibilities include assessments, care planning, resource mobilization, documentation in WellSky, and assisting with benefits applications.

Qualifications

• Associate degree in health and human services, social services, gerontology, or related field.
• High school diploma or GED with relevant aging services, social services, healthcare, or related experience.
• Demonstrate interest in and understanding of older adults' needs.
• Ability to communicate effectively with older adults, families, and partners.
• Maintain accurate and confidential participant records.
• Valid driver’s license and reliable transportation.

Responsibilities

• Take referrals, conduct home visits, and complete assessments to determine services.
• Develop individualized care plans outlining services and supports.
• Coordinate community resources to meet participant needs.
• Maintain participant files, records, and care plans with updates.
• Utilize WellSky for case notes and service documentation.
• Assist with applications and respond to correspondence related to benefits.
• Make referrals to Adult Protective Services and other resources as needed.
• Travel to homes, facilities, and government offices as required.
• Ensure confidentiality and collaborate with aging program staff.
