Examples of Duties

Under administrative direction, plans and administers a specific project or program for the Department of Aging. Serves as the administrative supervisor on projects or programs. Plans, assigns, supervises, and reviews activities of subordinate personnel. Monitors the project/program's operating budget and assists in the annual preparation of same. Designs, implements, and monitors procedures utilized in program evaluation. Develops and secures commitments from other City departments and agencies to provide services to program participants. Provides technical assistance or reviews program policies and procedures. Maintains current information and oversees the preparation of project status reports. Prepares financial and narrative reports and program applications. Communicates project/program status and information to municipal officials and/or public and private entities. May supervise employees assigned to the respective project. Performs other related duties as required.

Minimum Qualifications

A High School Diploma or GED is required. A Bachelor's Degree is required, those in Social Work, Social Services, Gerontology, Counseling, Human Services, Psychology Public Health, Health Services, or closely related field from a four-year accredited college or university are preferred; A Master's Degree is preferred. Two years of full time progressively responsible experience in government or private sector program administration is required. Three years of full time experience in human services is also required, working with the elderly and/or disabled adults is also preferred. (Substitution: Post graduate education in one of the above fields may substitute for work in government or the private sector and/or experience working with human services on a year-for-year basis) Must be able to operate a personal computer with Microsoft Office Suites. A valid State of Ohio Driver's License is required. Must be able to lift and carry 30 pounds. Must comply with the fingerprint-based records check under RCS 173.394. Strong verbal, written, oral, electronic, communication, and public speaking, planning, and organization skills are required.

Supplemental Information

Duties:

  • Respond to service referrals and inquiries from older adults and individuals with disabilities via phone, email, walk-ins, or home visits.
  • Complete comprehensive benefit screenings; explain benefit program eligibility and assist clients with application support.
  • Assist clients with applying for public assistance and cost-saving programs such as Medicaid, Medicare Savings Programs, Supplemental Security Income (SSI), Home Energy Assistance Program (HEAP), and Supplemental Nutrition Assistance Program (SNAP).
  • Conduct follow-ups to assess client needs, evaluate service effectiveness, and recommend appropriate support or services.
  • Maintain accurate and timely documentation using designated client database systems, including the Social Administration Management System (SAMS).
  • Supervise and support student interns, AARP trainees, and volunteers, including leading team meetings and delivering training sessions.
  • Collaborate with Department of Aging staff, other City of Cleveland departments, and community-based organizations to coordinate services and ensure client needs are met.
  • Build and maintain partnerships with external organizations to expand resources and services for older adults and individuals with disabilities.
  • Stay current on federal, state, and local programs, services, and policy updates related to aging and disability services.
  • Actively participate in the implementation of the Age-Friendly Cleveland Plan and other departmental initiatives.
  • Attend and co-facilitate community presentations and public education events to raise awareness of aging and disability-related resources.
  • Support departmental goals by completing monthly reports, audits, and achieving required billable service targets.
  • Collaborate closely with Aging and Disability Resource Center (ADRC) team members and partners in the Aging and Disability Resource Network (ADRN) to provide coordinated, holistic care.
