Aging Life Care Manager with Low-Income Clients

Join Your Elder Experts at Jewish Family & Children's Services as an Aging Life Care Manager dedicated to supporting low-income older adults in the Greater Boston Area. We are the largest not-for-profit aging life care program in the region, committed to guiding clients through aging challenges with expertise and compassion.

About the Role

The Care Manager will assess client needs, develop personalized care plans, connect clients with community resources, and provide direct support to help them achieve their goals. This includes coordinating services such as housing, benefits, medical care, and legal support, as well as accompanying clients to appointments and supporting their families and caregivers.

Key Responsibilities
  • Develop and implement personalized care plans based on thorough assessments.
  • Connect clients with critical community services, including affordable housing and healthcare.
  • Coordinate with other JF&CS programs and services.
  • Assist clients in accessing and navigating medical and community resources.
  • Transport and accompany clients to medical appointments.
  • Collaborate with family members and caregivers.
  • Maintain accurate case records and participate in team meetings.
Qualifications
  • Master’s degree in social work or related field, or 5+ years of relevant experience.
  • 2-4 years of experience working with older adults.
  • Knowledge of dementia, mental health, and community resources.
  • Valid driver’s license and access to a personal vehicle.
  • Proficiency in Microsoft Office and willingness to adopt new technology.
