Job summary

VOAC Colorado seeks a Service Coordinator to manage service provision and housing assistance for residents in the Affordable Housing Portfolio, serving seniors and persons with disabilities.

You will conduct needs-based assessments, outreach, case management, and connections to income resources while coordinating health, wellness, and educational programs to improve resident wellbeing and foster community. Full-time in Colorado, at $21–$22/hr.

Qualifications

• Bachelor's degree in human services, social work, or a closely related field or related experience.
• Minimum of one year working with senior citizens and/or people with disabilities.
• Willing to obtain first aid/CPR certifications.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient with Windows and databases.

Responsibilities

• Ensure residents' voices are heard and incorporated into programming.
• Facilitate referrals from Community Administrator, family, and residents.
• Assess resident needs via surveys, visits, and meetings; collect information.
• Coordinate services per HUD and VOAC guidelines.
• Provide information on services and program providers with multiple options.
• Encourage residents to arrange needed services to maintain independence.
• Monitor providers to ensure quality of care and cost effectiveness.
• Maintain up-to-date records on residents and service providers.
• Develop and facilitate resident groups and workshops.
