DESCRIPTION OF WORK

As an Aging Care Manager 2, you will receive case assignments involving general casework, intake and assessment. Your work with older adults will be ongoing, from initial contact with individuals, their families and professionals through care management responsibilities. Examples of duties include providing assistance to consumers with completion of program forms and making referrals of appropriate community services. You will have the opportunity to regularly coordinate services with physicians, community agencies, financial institutions, hospitals, nursing homes, and personal care homes. Work involves responding to client emergencies and protective services. Our ideal candidate will manage multiple tasks and display excellent attention to detail skills. You will be required to complete paperwork in a timely manner meeting all deadlines and input confidential client information into state databases.

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
  • On-call hours when required after training. Some out of county overnight travel may be required.
  • Telework: You will not have the option to telework in this position.
  • Salary:In some cases, the starting salary may be non-negotiable.

Required Experience, Training & Eligibility

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Schuylkill County. If no eligible candidates who live within Schuylkill County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

  • You must pass a background investigation.

  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
