An Aging Care Manager 2 provides consumers, family, and caregivers with information and directions related to the process of obtaining Waiver Services for the community, nursing facility (NF) placement, and/or personal care home (PCH) placement. Work involves completing a Functional Eligibility Determination (FED) assessment to determine eligibility and appropriate level of care for all ages of consumers residing in Lebanon County. This includes nursing facilities, individuals' homes, hospitals, or personal care homes. Collaboration with the supervisor will ensure the consumer is appropriately placed for their current level of need. Key to this position will be linking consumers, family member, and caregivers to available community services and resources in Lebanon County to provide continuity of care.
