Job summary

The Commonwealth of Pennsylvania, Northumberland County Area Agency on Aging, seeks an Aging Care Manager 2 to help older adults stay safe and independent in their communities. You will assess needs, coordinate services, and guide families through available programs to improve daily living.

You will work 8:30 AM to 4:30 PM, Monday through Friday, with no telework option. A bachelor’s degree in a social sciences field and relevant experience are required, with residency within PA preferred.

Qualifications

• Six months as an Aging Care Manager 1 or County Caseworker 1; or
• Successful completion of County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

Responsibilities

• Needs assessment: evaluate social contacts, daily living activities, cognitive functioning, physical health, and resources.
• Service coordination: connect older adults with agencies, hospitals, clinics, courts, and community organizations.
• Comprehensive reviews: conduct detailed assessments of medical, social, psychological, and housing conditions for care planning.
• Interagency collaboration: work with professional staff and community partners to discuss diagnoses and plans of treatment.
• Benefits guidance: assist older adults in managing income, accessing Social Security and Medicare, and understanding relevant legislation.
• Case documentation: prepare social studies, case histories, reports, and records.

As an Aging Care Manager 2 with the Northumberland County Area Agency on Aging (AAA), you help older adults overcome challenges and stay safe in their communities. This position supports individuals and families by linking them to programs that make daily life easier. You make a positive impact through careful planning and thoughtful guidance. Your work strengthens the well-being of those who rely on compassionate care.

This role centers on assessing needs and coordinating services that enhance the quality of life for older adults. As an Aging Care Manager 2, you will perform the following duties:

Work Schedule and Additional Information

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
• Telework: You will not have the option to telework in this position.

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
