DESCRIPTION OF WORK

Information and Referral is the gateway to aging programs and services offered through the Delaware County Office of Services for the Aging. As an Information & Referral (I&R) Specialist, also known as an Aging Care Manager 2, you will be providing exceptional customer service to seniors, caregivers, family members, and professionals as you provide valuable information and resources. Work also involves researching community resources beneficial to seniors through outreach efforts to other community organizations and maintaining awareness of changes to keep resources current. You will have the opportunity to work closely with other agencies and providers to ensure appropriate services are offered to consumers. Additional responsibilities include:

  • Scheduling assessments for seniors who may need help in their home
  • Taking report of suspected abuse, neglect, exploitation, or abandonment for the Protective Service Unit
  • Assisting consumers complete applications for other benefits and entitlements
  • Attending meetings and participating in trainings

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary:In some cases, the starting salary may be non-negotiable.

Required Experience, Training & Eligibility

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • You must be able to perform essential job functions.
