After successful completion of a six-month probationary period per union agreement, the selected candidate will be promoted to Aging Care Manager 2.
As an Aging Care Manager 1 you will manage a caseload and work with consumers, their families, and other agencies to utilize all resources available to provide assistance to consumers. Upon completion of training, you will be expected to exercise initiative and independent judgment in discharging assignments, seeking help with more unusual or difficult cases.
As an Aging Care Manager, you will develop and manage care plans through careful evaluation of consumers’ personal, social, and economic situation. This includes obtaining a variety of information about each consumer, assisting consumers in managing incomes to maximize benefits, consulting with other professional staff in specialized program areas, and working in close cooperation with other social agencies, health care providers, courts, and community resources. You will also complete training in preparation to participate in protective services and Medicare benefits counseling functions.
Your work will be essential for aiding consumers and their families in achieving a more satisfactory adjustment to specific problems or situations as well as effectively utilizing all available resources.
