Job summary

St Jude's Health Care Group is seeking a Lifestyle Coordinator to design and oversee a comprehensive Lifestyle program aligned with resident preferences and capabilities. The role focuses on maximizing residents' abilities and enhancing social, cultural, and emotional connections within the home.

You will mentor staff, liaise with health professionals and families, and ensure activities reflect each resident's interests while maintaining safety and policy compliance.

Qualifications

• Ability to relate to elderly clients with empathy and work with health professionals.
• Experience in assessment and equipment prescription in aged care.
• Experience in development and delivery of therapy-based programs.
• Commitment to being a productive and positive team member.
• Excellent interpersonal, communication and time management skills.
• Knowledge of community based programs with flexible approach.

Responsibilities

• Establish positive communication with clients and families to produce activity assessments and care plans.
• Develop, implement and evaluate individual social and leisure care plans; mentor the team.
• Identify entertainment options and incorporate into monthly program; promote engagement.
• Build relationships with support staff, nurses and volunteers to support holistic wellbeing.
• Ensure lifestyle coordination aligns with policies and addresses risks or hazards.
