Job summary

Heart of Hospice, part of LHC Group within the Optum family, seeks an Afterhours Registered Nurse to provide comprehensive hospice care after regular hours, including nights and weekends. Responsibilities include tele-triage, crisis response, and after-hours visits in Louisiana.

Qualified candidates hold an unrestricted RN license in Louisiana, a valid driver's license with reliable transportation, CPR certification (or willingness to obtain within 90 days), and at least two years of RN

Qualifications

• Current unrestricted RN licensure in the state of practice.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit.
• Respond to after-hours crisis calls from patients and families.
• Perform initial nursing evaluation within 48 hours of referral when assigned.
• Conduct after-hours admission, death, recertification, and routine visits.
• Assess and reassess patient/family needs every 15 days.
• Document problems, goals, interventions, and responses to hospice care.

As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends.

Primary Responsibilities

• Able to triage patient needs over the phone until a nurse can provide an in-person visit
• Responds appropriately and with urgency to crisis calls from patients and families afterhours
• Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned
• Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits
• Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care

Required Qualifications

• Current unrestricted RN licensure in the state of practice experience
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

State Specific Requirements

• LA: a minimum of two years full-time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• 1+ years of clinical experience

The hourly pay for this role will range from $37.40 to $56.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
