Job summary

UnitedHealth Group's Heart of Hospice (LHC Group) seeks an Afterhours Registered Nurse to deliver hospice care to patients and families after regular hours, including nights and weekends. Responsibilities include triaging needs by phone, urgent crisis response, initial nursing evaluations within 48 hours, and afterhours visits such as admissions, death, and recertifications.

You will assess physical, psychosocial, and environmental needs and document care plans.

Qualifications

  • Current unrestricted RN licensure in state of practice.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
  • MS state requirement: at least one year RN experience; three years LPN may substitute.

Responsibilities

  • Triage patient needs over the phone until an in-person visit by a nurse.
  • Respond to afterhours crisis calls from patients and families with urgency.
  • Perform initial nursing evaluation within 48 hours of referral when assigned.
  • Conduct afterhours visits including admissions, death, recertification and routine visits.
  • Assess and reassess patient/family needs every 15 days and adjust care plans.
  • Document problems, goals, interventions and patient/family responses to care.
  • Provide care in diverse home situations irrespective of patient demographics.

As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends.

Primary Responsibilities:
  • Able to triage patient needs over the phone until a nurse can provide an in-person visit
  • Responds appropriately and with urgency to crisis calls from patients and families afterhours
  • Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned
  • Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits
  • Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:
  • Current unrestricted RN licensure in the state of practice experience
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
State Specific Requirements: (remove as applicable)
  • MS: At least one-year full-time experience as an RN. However, three (3) years full-time clinical experience in a healthcare setting as an LPN may be substituted for the one-year full time experience as an RN
Preferred Qualifications:
  • Current CPR certification or ability to complete within 90 days of hire
  • 1+ years of clinical experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $63,376 to $95,064 annually based on full-time employment. We comply with all minimum wage laws as applicable.
