Job summary

Optum, through At Home Hospice, is seeking an RN to provide home health and hospice services in the Round Rock, Texas area. The role includes triage over the phone, afterhours crisis response, and initial nursing evaluations to determine hospice eligibility within 48 hours of referral.

Travel across Williamson and Travis Counties is required. Active Texas RN license, CPR, and at least one year of clinical experience are necessary; hospice experience is preferred.

Responsibilities

• Triage patient needs over the phone until a nurse can visit in person.
• Respond with urgency to crisis calls after hours from patients and families.
• Conduct initial nursing evaluation within 48 hours of referral for hospice eligibility.
• Perform afterhours visits including admission, death, recertification and routine visits.
• Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family response to hospice care.

Requirements

Primary Responsibilities

• Able to triage patient needs over the phone until a nurse can provide an in-person visit.
• Responds appropriately and with urgency to crisis calls from patients and families afterhours.
• Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned.
• Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits.
• Identifies the patient/family’s physical, psychosocial, emotional, and environmental needs and reassesses them as needed, no less than every fifteen (15) days.
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care.

Required Qualifications

• Current unrestricted RN licensure in the state of practice experience
• Current CPR Certification
• Minimum One year of clinical experience
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

• Current unrestricted RN licensure in the state of operation
• Current CPR certification.
• Current Driver’s License and vehicle insurance, and access to a dependable vehicle or public transportation
• Minimum one year of experience

Pay Range

$71,677 - $107,516 annual total cash target pay

$34.46 - $51.69 visit point

$41.35 - $62.03 hourly rate

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of direct and indirect patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
