Job description

After Hours Visit Licensed Vocational Nurse (LVN)

12am-8:30am or 5pm-1am


The posted compensation range of $35.70 - $49.78/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.


POSITION SUMMARY:

The Shared Services After Hours Visit LVN is responsible for providing skilled nursing visits to patients and their families as requested by Intella Triage. Documents observations and interventions related to the patient’s environment, physical, mental, emotional and social status. Communicates care with other members of the Interdisciplinary Group (IDG) or team, including patient’s physician.


AREAS OF RESPONSIBILITY:
  1. Provides skilled nursing observation of the patient’s condition, problems, care interventions, patient and family outcomes, and evaluation of these, and documents this in the electronic record.
  2. Communicates patient updates to Intella Triage or the Triage Nurse and the case manager, informing him/her of significant changes in the patient’s condition and needs, and interventions made. Documents these communications.
  3. Contacts patient’s physician when necessary.
  4. Demonstrates skills of a Licensed Vocational Nurse, including management of subcutaneous medication pumps.
  5. Maintains in-dated and adequate stock of medical supplies and agency forms.
  6. Able to discuss the overall history, philosophy and services of Pathways Home Health and Hospice.
  7. Provides and documents skilled nursing observation of the patient’s condition, problems, care interventions, patient and family outcomes, and evaluation of these in the electronic record.
  8. Accesses electronic device during patient visit and documents during or immediately after the visit or per regulation.
  9. Reports regularly to the Manager of Clinical Services - After Hours regarding care provided, and immediately notifies him/her when patient condition is found to be unstable and there is a need for changes in current plan of care.
  10. Supports, instructs and educates the patient, family, caregivers, and facility staff.
  11. Attends educational in-service trainings, and other agency meetings as requested by manager.
  12. Maintains the dignity of the dying patient and supports the patient’s and family’s unique spiritual and cultural beliefs.
  13. Provides holistic, family-centered care across the treatment settings to improve the quality of life.
  14. Knowledgably discusses and initiates non-pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress.
  15. Participates in the Pathways quality assessment performance improvement program and in-service education related to areas of practice as appropriate.
  16. Meets productivity standards as established by manager.
  17. When in a facility, adheres to “Pathways Standards of Care and Practice in Facilities” or equivalent document.
  18. Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facility preference at the time of visit.
  19. Maintains timely communication with patient, family, and facility staff regarding all aspects of patient care.
  20. Demonstrates awareness of and adherence to the safety, infection control and other policies of the facility.
  21. Maintains and updates level of knowledge and skills required to provide quality nursing care.
  22. Updates and maintains medication list. Monitors for side effects and communicates these. Effectively instructs patient, family, and/or staff use of medications and reconciles medication records.
  23. Effectively communicates with and uses the hospice prescription plan, formulary/preferred drug list, and general hospice medication coverage philosophy.
  24. Adheres to all agency policies and procedures, including updates, as communicated in written, spoken, and electronic forms.
  25. Promotes a culture of service when representing Pathways by at all times treating internal and external contacts with respect, remaining professional, communicating clearly, preventing conflict, following through on commitments, and actively listening. Positively promotes Pathways by offering assistance and information.
  26. Demonstrates all skills of a Licensed Vocational Nurse, including those skills required by the agency through competency testing.
  27. Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways.

QUALIFICATIONS:
  1. California LVN license.
  2. Preferred “IV Blood Withdraw” on discipline license.
  3. Graduate of an NLN accredited nursing school.
  4. Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary team setting. Excellent nursing assessment, communication, and problem-solving skills.
  5. Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
  6. California driver’s license and automobile insurance with minimum Bodily Injury Liability coverage Insurance per agency policy.
  7. Current CPR Certificate– American Heart Association “BLS Health Care Provider” requested upon renewal.
