By The Bay Health is seeking an After-Hours Licensed Vocational Nurse (LVN) to work under a Registered Nurse to deliver skilled patient care as outlined in the care plan. The office is located in Larkspur, CA, serving Marin County with additional support to Sonoma and Napa Counties as needed.

The role requires completion of an LVN program and a California LVN license, with at least one year of LVN experience.

Competenze

• Completion of a Licensed Vocational Nursing Education Program from an accredited school.
• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
• Minimum of one year LVN experience; home health/hospice experience preferred but not required.

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

We are seeking anAfter-Hours Licensed Vocational Nurse (LVN) that will work under the supervision of a Registered Nurse to provide skilled patient care services as outlined in the written plan of care.

Schedule:

Thursday & Friday; 3:30pm - 12:00am

Saturday & Sunday; 9:00am - 5:30pm

Office Location:

Larkspur, CA

Coverage Area:

Marin County with additional coverage to Sonoma & Napa Counties as needed

Essential Duties & Responsibilities include

(but are not limited to):

• Assist a registered nurse in providing direct care
• Initiate communication with physicians and managers, IPG members, HHA’s and other health care providers as needed to facilitate optimal care
• Instruct primary caregivers, patients, volunteers and employed caregivers in care provision based on the Plan of Care and as requested by the Primary Nurse or RN Manager. Note: The LVN receivesdirection from the RN on-site and may not alter the patient/family plan of care
• Attend patient conference and participate in discussion of issues to facilitate optimal care

Education/Experience:

• Completion of a Licensed Vocational Nursing Education Program from an accredited school
• Minimum of one year of LVN experience preferably in an acute care setting. Experience in a home health/ hospice setting preferred but not required

Certificates, Licenses, Registrations:

• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners

Pay Range:

$50.50 - $59.50 hourly _ + shift differential included _
