Qualifications

• RN licensed in Illinois (IDFPR) and BLS certified (AHA).
• College diploma in Nursing and valid driver’s license.
• Proof of auto insurance required.

Responsibilities

• Provides on-call coverage and patient visits as assigned for home health.
• Coordinate care, triage referrals, admissions and follow-up visits.
• Maintain accurate clinical records per policies and regulations.
• Communicate care plans to patients, families and providers.
