• Provide home-based support for individuals with I/DD in AFL settings.

Responsibilities

• Support individuals with disabilities in home settings, following their goals.
• Assist with meals, daily living, transportation and community activities.
• Ensure home safety and privacy while implementing the plan.
• Attend supervision meetings and communicate needs with the supervisor.
• Collaborate with multidisciplinary team for crisis assistance and training.
