Position Summary
As an Assistant Manager, this position is responsible for setting a good example for co-workers, maintaining good attendance, and possessing a positive attitude. It assists developmentally, physically, and cognitively disabled adult individuals by providing personal care, self‑care training, implementation of behavior management plans, and support during social/recreational opportunities, either in the resident’s home, the Connections Program, or the community at large. The role includes quality assurance for residents, treating them with respect, affirming their rights, and providing a safe environment.
Essential Functions and Responsibilities
- Assists with activities such as bathing, dressing, changing bed linens, cleaning, and personal hygiene.
- Assists with food purchases, preparation, portion determination, and follows special prescribed diets.
- Assists residents/participants in and out of bed, automobile, or wheelchair, to the lavatory, and up and down stairs.
- Accompanies residents on shopping trips, guides and assists them in purchasing personal items, and is accountable for resident funds.
- Assists in following and administering therapeutic plans and activities such as physical exercises, occupational arts and crafts, recreational games, and educational opportunities.
- Performs verbal and non‑violent physical crisis intervention with agitated or disruptive residents/participants to prevent injury.
- Administers prescribed medications under written direction of a Physician or as directed by the Supported Living Manager.
- Observes and documents behaviors such as speech production, emotional responses, feeding patterns, and toileting habits.
- Attends to and monitors routine health‑care needs of residents and participants under medical supervision.
- Transports residents to/from appointments.
- Reports signs, symptoms, and/or needs of residents to appropriate medical personnel.
- Takes prescriptions to pharmacy for filling and communicates new orders to staff and agency RN.
- Reviews medication administration records and reports discrepancies to agency RN.
- Accompanies ambulatory and non‑ambulatory residents/participants outside home, serving as guide, companion, and aide.
- Engages residents/participants in recreational, educational, social, or spiritual activities such as reading aloud, writing, making crafts, word processing, or playing games.
- Maintains records of services performed and of apparent condition of resident/participant.
- Works with the Supported Living Manager to develop and coordinate behavior plans and outcome measurements for residents/participants.
- Maintains a cooperative relationship with families and guardians of residents/participants, team members, and community agencies.
- Communicates with RN to determine appointment needs, report outcomes, complete documentation, and assist with maintaining files.
- Maintains residents’ rights by ensuring quality care, monitoring health, safety, and welfare, reporting necessary changes, following behavior plans, and reporting violations to management.
- Quality Assurance through DQA by familiarizing self with HFS 88, resident rights, grievance procedures, fire safety, and state licenses.
- Manages compliance documentation including emergency procedures, fire evacuation plans, fire extinguisher testing, furnace filter changes, thermostat batteries, water temperature adjustments, and emergency phone numbers.
- Ensures all charting is up‑to‑date and signed off in resident binders (progress notes, activity logs, goal sheets, behavior tracking, medical records, range of motion, weight charts, menus, blood pressure tracking).
- Responsible for end‑of‑month paperwork (pulling and copying progress notes, goal sheets, behavior tracking, med records, etc.).
- Manages residents’ medications: ensures meds are in stock, administered without error, reorders, checks expiration dates, and stores according to DQA standards.
- Ensures each resident has an adequate supply of clothes for the season, marks clothing with resident initials, and maintains personal inventory.
- Manages resident money and maintains accurate accounting of all resident monies.
- Ensures grocery budget is managed and groceries last for the month.
- Ensures residents always have access to their money.
- Tracks all receipts for money spent by a resident.
- Ensures resident accounts are ready and accurate for monthly audit.
- Creates and maintains a cleaning checklist for resident’s home; ensures the house is clean and orderly daily.
- Represents Opportunity Inc. in a professional manner at all times.
- Supports supervisor and leadership for success at all times.

Qualifications
- Must be able to pass an essential function evaluation.
- The employee must frequently lift, support, and/or move up to 75 pounds and occasionally 100 pounds.
- Must be caring, compassionate, and want to make a difference in residents/participants lives.
Certificates and Licenses
- PCW certification.
- Transportation Department Drivers Training.
- Valid driver’s license with good driving record may be required based on agency needs.
- Opportunity Inc. will provide all necessary training.
- Previous PCW certification, CNA license, or other required training certificates are transferable.
