Summary

Responsible for planning, providing, and coordinating all aspects of direct patient care in accordance with clinic policies, maintaining high-quality care in a safe environment. The therapist collaborates with other healthcare team members and assumes responsibility for services delivered by Physical Therapist Assistants and Rehab Techs/Aides. May also orient staff and students, and assume duties of the Clinic Director in their absence.

Essential Duties And Responsibilities
  • Provide high-quality patient care and assessment according to standards and policies.
  • Perform patient evaluations, develop individualized care plans, and set feasible goals.
  • Administer treatments as per physician’s orders and care plans.
  • Evaluate and revise treatment plans based on effectiveness.
  • Communicate with patients, families, caregivers, and healthcare team members to optimize care.
  • Assess educational needs and promote patient and family understanding.
  • Apply clinical techniques and procedures appropriately.
  • Plan and execute discharge plans.
  • Assess safety risks and implement precautions.
  • Follow security and safety standards.
  • Lead by delegating, organizing, and educating staff.
  • Participate in quality improvement initiatives.
  • Use patient scheduling efficiently to meet productivity standards.
  • Complete documentation timely and accurately using AgileRPM.
  • Travel to other local clinics as needed.

Qualifications
  • Master’s or Doctorate degree in Physical Therapy.
  • Current Physical Therapist license, registration, and/or certification as per state regulations.
Physical Demands
  • Must be able to stand, walk, sit, reach, climb, balance, kneel, crouch, or crawl regularly.
  • Lift up to 25 pounds frequently and occasionally move more than 100 pounds with assistance.
