Clear Concepts, Inc. seeks an experienced Advanced Registered Nurse Practitioner (ARNP) in Washington to join a patient-centered, team-based care setting.

This permanent, full-time role collaborates with physicians, PAs, ARNPs, and nursing staff to deliver comprehensive care within the PCMH model. The position emphasizes clinical assessments, patient education, care coordination, and adherence to evidence-based practices, with relocation assistance available and a strong focus on population

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority in Washington State.
• Experience in broad primary care clinic settings.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on care.
• Develop and implement teaching plans for patient self-management.
• Use independent clinical judgment based on evidence data.
• Maintain timely, accurate chart documentation (within 48 hours).
• Complete fee slips with matching diagnosis and codes.
• Meet productivity expectations (min 3,400 encounters/year).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk populations and monitor panel health.
• Explain procedures and plans to patients prior to execution.
• Participate in quality management and department development.
• Support clinical education of student nurse practitioners.
• Maintain required annual training (HIPAA, diversity, BBP, Harassment).

Position Overview

We are seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization committed to team-based care. This role operates within the Patient Centered Medical Home model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Key Responsibilities

• Perform advanced clinical assessments and manage illnesses and conditions within ARNP scope of practice
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time)
• Complete fee slips with appropriately matching diagnosis and procedure codes
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent)
• Provide care coordination through daily huddles as part of the PCMH model
• Identify at-risk patient populations and monitor panel-specific population health
• Empower patients through self-management education and discharge planning
• Advocate for patients by explaining all procedures and treatment plans prior to execution
• Participate in quality management, peer review, and departmental practice development
• Support clinical education of other health professionals including student nurse practitioners
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment)

Qualifications

• Graduate of an accredited ARNP program (required)
• Current RN and ARNP licensure in the State of Washington (required)
• Prescriptive authority from Washington State (required)
• Experience practicing in a broad primary care clinic setting (required)
• Experience in minor surgical techniques (preferred)
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation
