Top Echelon Software, LLC seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization in Washington. This role participates in a PCMH team to deliver high-quality care.

Requires WA RN/ARNP licensure, prescriptive authority, and broad primary care experience. Relocation assistance and a comprehensive benefits package are provided. The position is full-time with a competitive salary.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).
• Strong nursing process knowledge: assessment, planning, implementation, evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on care management.
• Develop and implement individualized teaching plans for health goals.
• Apply independent clinical judgment based on evidence-based data.
• Maintain chart documentation within 48-hour timeline (95%+).
• Complete fee slips with matching diagnosis and procedure codes.
• Meet productivity: minimum 3,400 encounters per year (FT).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panels’ health outcomes.
• Empower patients through self-management education and discharge planning.
• Explain procedures and plans to patients prior to execution.
• Participate in quality management, peer review, and practice development.
• Support education of student nurse practitioners and other staff.
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, Harassment).

Position Overview

We are seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization committed to team-based care. This role operates within the Patient Centered Medical Home model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Key Responsibilities

• Perform advanced clinical assessments and manage illnesses and conditions within ARNP scope of practice
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time)
• Complete fee slips with appropriately matching diagnosis and procedure codes
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent)
• Provide care coordination through daily huddles as part of the PCMH model
• Identify at-risk patient populations and monitor panel-specific population health
• Empower patients through self-management education and discharge planning
• Advocate for patients by explaining all procedures and treatment plans prior to execution
