Acloché LLC is seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based primary care team in Washington. The role operates within a PCMH model, collaborating with physicians, PAs, ARNPs, and nurses to deliver comprehensive care across a diverse patient population.

The successful candidate will perform advanced clinical assessments, manage illnesses within ARNP scope, develop patient teaching plans, and ensure timely charting.

Qualifications

• Must be graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority in Washington.
• Experience practicing in broad primary care clinic setting.
• Experience in minor surgical techniques preferred.

Responsibilities

• Perform advanced clinical assessments within ARNP scope and manage illnesses.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving optimal health.
• Apply sound clinical judgment using evidence-based data for patient decisions.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Complete fee slips with correctly matched diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters per year, FT).
• Provide care coordination through daily huddles as part of the PCMH model.

Position Overview

We are seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization committed to team-based care. This role operates within the Patient Centered Medical Home model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Key Responsibilities

• Perform advanced clinical assessments and manage illnesses and conditions within ARNP scope of practice
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time)
• Complete fee slips with appropriately matching diagnosis and procedure codes
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent)
• Provide care coordination through daily huddles as part of the PCMH model
• Identify at-risk patient populations and monitor panel-specific population health
• Empower patients through self-management education and discharge planning
• Advocate for patients by explaining all procedures and treatment plans prior to execution
• Participate in quality management, peer review, and departmental practice development
• Support clinical education of other health professionals including student nurse practitioners
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment)

Qualifications

• Experience practicing in a broad primary care clinic setting (required)

• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation
