Job summary

Halifax Health in Florida is seeking a Day (United States of America) Advanced Practice Registered Nurse (APRN) – Hospice to join our multi-disciplinary team. The APRN will independently assess patients, provide advanced nursing care, and coordinate symptom management for adults and geriatric patients in a hospice setting.

This role requires an MS in Nursing, Florida licensure, and collaboration under a practice agreement.

Qualifications

• Graduate of an accredited program in nursing, MS in nursing is required
• Licensed by the State of Florida
• Professional knowledge of nursing theory and practice
• Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and management of acute chronic patient problems

Responsibilities

• Evaluate patient history and perform physical examinations to determine needs and treatment
• Provide nursing care and health teaching to adult and geriatric patients in a hospice setting
• Collaborate within a multidisciplinary health team and adhere to the Florida collaborative practice agreement

The Nurse Practitioner is an independent health care provider who, working collaboratively within a multi-disciplinary health team, is responsible for patient evaluation and care through the performance of pertinent history and physical examination, assessment of needs and delivery of appropriate treatment. Provides nursing care and health teaching to an assigned group of adult and geriatric patients in an observation setting. All duties and responsibilities are accomplished according to the collaborative practice agreement required by the State of Florida.
