Endeavor Health is hiring an Advanced Practice Provider (APN/PA) for Primary Care in Mount Prospect, IL. This role supports a family medicine department with a 32-hour work week and a mix of weekdays; on-site at 199 W. Rand Rd.

Ideal candidates hold IL APN/PA licensure with DEA, a master's in a related field, BLS/CPR, and at least 2 years of primary care experience. The position offers a competitive hourly pay range and rotating Saturday shifts.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in IL
• Federal DEA and Illinois Controlled Substance License required
• Education: Master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Minimum of 2 years APN/PA experience in Primary Care setting

$57.31 - $72.36 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

• Full Time/Part Time: Full Time, 32 hour week
• Hours: Monday - Friday, between the hours of 7:00am - 5:00pm
• Rotating Saturday's, 8:00am - 12:00pm
• Rotating Call - Approx. one week/year

Job Description

Exciting opportunity to join Endeavor Health in the Primary Care Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications:
