Job summary

OSF HealthCare in Illinois seeks an experienced advanced practice provider to join an established primary care practice. You will assess patients, diagnose based on data and guidelines, and establish care plans with evidence-based interventions.

You will educate patients and families, coordinate care, and contribute to a patient-centered team with standard schedules and no weekend on-call requirements.

Qualifications

• Master’s degree or higher in nursing or physician assistant studies from an accredited program.
• Current Illinois license as a nurse practitioner or physician assistant.
• Certification by the American Nurses Credentialing Center (ANCC) or the NCCPA.

• Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific and current research.
• Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care and current research.
• Provide education and counseling to patients, families and groups.
• Clinic hours are Monday – Friday 8am-5pm; excellent work/life balance with no call or weekends.

Expected pay for this position is $55.20 - $63.65/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.

• Clinical base hourly pay range for this position: $55.20-63.65 hr. This is a salaried position.

Qualifications

REQUIRED QUALIFICATIONS

• Master’s degree or higher in nursing or physician assistant studies from an accredited program
• Current Illinois license as a nurse practitioner or physician assistant
• Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
• Minimum of two years of experience as an APP in primary care or a related field is preferred
• Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
• Proficiency in electronic medical record (EMR) systems
