FIRST CHOICE COMMUNITY HEALTH CENTERS INC is seeking a full time Advance Practice Provider (NP/PA) for various locations in North Carolina. This position involves providing direct patient services such as assessment, diagnosis, treatment implementation, and collaboration with physicians as part of the Patient Centered Medical Home team.

The ideal candidate will hold a PA/NP degree, have a current North Carolina license, and possess solid experience in primary care, demonstrating a commitment to community health.

Qualifications

• At least one year of experience in a primary care setting.
• Minimum of 1-2 years of experience in a clinical setting.
• Strong commitment to practicing in a community health setting.

Responsibilities

• Obtain complete medical history from patients.
• Prescribe medications and explain treatment.
• Develop therapeutic and educational plans.
• Consult with physicians and other healthcare team members.
• Maintain patient records to comply with standards.
