Job description

Summary

The Physician Assistant (PA) or Nurse Practitioner (NP) provides direct detainee care, including evaluations, minor procedures, and medication prescriptions. The incumbent coordinates referrals, ensures policy compliance, mentors APPs, and reports to the Supervisory APP or Clinical Director if no Supervisory APP is on site. This position is open only to USPHS officers or Call to Active Duty (CAD) candidates.

Duties (O-3 & O-4 Billet)

• Practice in conformance with established criteria, guidelines, and agency procedures to enhance the well-being of individuals.
• Perform comprehensive history and physical evaluations, behavioral health care, and minor outpatient surgical procedures for detainees as required by IHSC policy or medical complaint.
• Coordinate health-care needs, including referrals to off-site consultants.
• Take appropriate infection-control actions in accordance with IHSC policy.
• Complete required and assigned training.
• Attend on-site, regional, and national meetings as required.
• Participate in the APP Peer Review process at O-4 level.
• Perform other duties and responsibilities as assigned.

Duties (O-5 Billet)

• Provide direct care to detainees via scheduled appointments, emergency, or urgent care visits.
• Consult with the physician or Clinical Director for complex medical and/or psychiatric patients as required by IHSC policy.
• Provide infirmary care, including admission, discharge, and daily rounds for detainees requiring higher levels of medical care.
• Perform comprehensive history and physical evaluations, behavioral health care, and minor outpatient surgical procedures as required.
• Work within and perform functions as outlined in the IHSC APP Scope of Practice and the Controlled Prescribing Scope of Practice.
• Apply knowledge of drugs and therapeutics when prescribing medication.
• Interpret diagnostic and laboratory results, TSTs, and manage DOT for tuberculosis.
• Take appropriate infection-control actions, adhere to universal precautions, and dispose of sharps properly.
• Serve as medical escort of detainees for domestic or overseas travel when needed.
• Utilize and document interpretation or translation services for LEP detainees or those with communication disabilities.
• Maintain flexibility for sudden schedule changes.
• Provide patient health education verbally or via handouts.
• Document detainee care visits in the required health record in compliance with IHSC policy.
• Maintain patient confidentiality.
• Complete required training and assignments.
• Serve as Acting Senior Supervisory APP or Acting Regional APP.
• Facilitate and mentor APP onboarding and training.
• Attend on-site, regional, and national meetings as required.
• Participate in the APP Peer Review process.
• Perform other duties and responsibilities as assigned.

Conditions of Employment

• US citizen.
• Successfully pass background investigation, drug screen, and (for males born after 12/31/59) certify registration with Selective Service.
• Meet all requirements by the closing date of the announcement.
• Upon appointment, officers must serve an 18 to 24 month commitment to IHSC.
• Must not have a current Assignment Pay Agreement; if one exists, it must be fulfilled within 90 days of application.
• Applicants must be current USPHS Commissioned Corps Officers or USPHS CAD candidates.

Education and Experience

Required Knowledge, Skills, and Abilities (O-3 & O-4 Billet)

• O-3: New graduate or no clinical experience; O-4 upgrade depends on performance.
• O-4: One year of clinical experience as PA or NP; may be an officer in an O-4 billet or approved by the Deputy Medical Director.
• Meet minimum education and certification requirements for PA or NP.
• PA and NP licensure and national certification as specified.
• Maintain current licensure, DEA, CPR/BLS, and continuing education.
• Preferred knowledge: experience in correctional health care, IHSC policies, NCCHC, PBNDS, public health preparedness, infection control, TB management, ACLS, DEA Buprenorphine waiver.

Required Knowledge, Skills, and Abilities (O-5 Billet)

• More than 4 years of full-time clinical experience as PA or NP, or approval by IHSC Medical Director.
• Team lead or supervisory experience.
• Licensure and national certification for PA or NP as required.
• Maintain current licensure, DEA, CPR/BLS, and continuing education.
• Preferred knowledge: correctional health care experience, CCHP certification, IHSC policies, NCCHC, PBNDS, public health preparedness, TB management, ACLS.

This position is located within the Department of Homeland Security (DHS), Immigration and Customs Enforcement (ICE), Enforcement and Removal Operations (ERO), ICE Health Service Corps (IHSC), Office of Deputy Assistant Director of Clinical Services. This is a non-supervisory position.
