Job Overview

The Advanced Practice Provider (APP) will deliver holistic care within the Department of Family Medicine, focusing on the Whole Person Pain Service (WPPS) and primary care clinics. The WPPS provides virtual chronic pain management for patients on long-term opioids, serving as a central hub for pain care in the department. The APP's responsibilities include chronic pain management, care coordination, and ongoing coverage at current clinical sites.

Key Responsibilities
  1. Whole Person Pain Service Clinical/Direct Patient Care (0.20-0.25 CFTE)
  2. Provide virtual care for chronic pain patients, including intake, follow-up, and group visits.
  3. Participate in chart reviews and clinical coding activities.
  4. Collaborate with medical staff and specialists for complex cases.
  5. Coordinate patient care with other health professionals.
  6. Triage calls and messages for the WPPS.
  7. Utilize opioid registries for patient outreach and risk assessment.
  1. Whole Person Pain Service Care Management (0.3-0.50 FTE)
  2. Manage opioid registries and conduct high-risk patient case conferences.
  1. Float APP Clinical/Direct Patient Care (0.25-0.50 CFTE)
  2. Deliver primary care for diverse patient populations across CU hospitals and clinics.
  3. Handle acute care needs and collaborate on complex patient management.
  4. Participate in chronic disease management and team-based care.
Additional Responsibilities
  • Maintain faculty eligibility and engage in professional development.
  • Attend conferences and training to stay current and enhance skills.
Work Location

Hybrid schedule with occasional meetings at Anschutz campus in Aurora, CO. Candidates must reside in Colorado at the time of hire.

Qualifications

Minimum Qualifications:

  • Masters or Doctor of Nursing Practice degree or NCCPA-approved PA program graduation.
  • Colorado RN and NP or PA licensure.
  • Completion of 1,000+ hours for full prescriptive authority and DEA registration.
  • Medicare/Medicaid credentials, unrestricted DEA, CPR certification.
  • Family or adult nurse practitioner certification (if applicable).

Preferred Qualifications:

  • 3+ years of practice in a Patient-Centered Medical Home or primary care setting.
  • Experience with Epic or similar EHR systems.
  • Cultural humility and bias minimization skills.
