Job summary

MarinHealth Nephrology is seeking an Advanced Practice Provider (NP/PA) to join a collaborative team of four nephrologists. The APP will provide comprehensive care for patients with chronic kidney conditions, working closely with our nephrologists and multidisciplinary care teams across the continuum of care.

The successful candidate will have CA NP/PA licensure, DEA registration, BLS, and a strong interest in nephrology.

Qualifications

• Graduate of an accredited NP or PA program.
• Current, unrestricted California NP or PA license.
• Current prescribing/furnishing authority and DEA registration.
• BLS certification; ACLS preferred.
• Ability to practice independently within scope while collaborating with physicians.
• Experience in nephrology or related fields preferred.

Responsibilities

• Evaluate and manage CKD, AKI, hypertension, electrolyte disorders, and ESKD.
• Perform histories and physical examinations and develop individualized plans.
• Order/interpret labs, imaging, and other studies.
• Prescribe/manage medications per protocols.
• Monitor progression and adjust care plans.
• Educate patients and families on kidney disease, dialysis, and lifestyle changes.
• Coordinate care with nephrologists, PCPs, specialists, dialysis providers and care team.
• Support continuity of care including hospital follow-up and referrals.
• Recognize changes requiring physician escalation.
• Document timely and accurately; participate in quality initiatives.

• The APP will provide comprehensive care for patients with acute and chronic kidney conditions, working closely with our nephrologists and multidisciplinary care teams across the continuum of care.

Key Responsibilities

• Evaluate and manage new and established patients with chronic kidney disease, acute kidney injury, hypertension, electrolyte disorders, and end-stage kidney disease.
• Perform comprehensive histories and physical examinations and develop individualized assessment and treatment plans.
• Order and interpret appropriate laboratory, diagnostic, and imaging studies.
• Prescribe and manage medications in accordance with applicable scope of practice, licensure, and clinical protocols.
• Monitor disease progression and response to treatment, adjusting care plans as clinically appropriate.
• Educate and counsel patients and families regarding kidney disease, medications, dialysis, lifestyle modifications, and treatment options.
• Coordinate care with nephrologists, primary care physicians, specialists, dialysis providers, and other members of the healthcare team.
• Support continuity and transitions of care, including hospital follow-up and referrals.
• Recognize changes in clinical condition requiring physician evaluation or escalation of care.
• Maintain timely, accurate clinical documentation and participate in quality, safety, and practice improvement initiatives.

Position Requirements

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current, unrestricted California NP or PA licensure and applicable national board certification.
• Current prescribing/furnishing authority and DEA registration, as required for the role and professional discipline.
• Current BLS certification; ACLS preferred based on clinical responsibilities.
• Strong clinical assessment, diagnostic, critical-thinking, communication, and interpersonal skills.
• Ability to practice independently within authorized scope while collaborating closely with physicians and the broader care team.
• Experience in nephrology, internal medicine, hospital medicine, dialysis, cardiology, or a related specialty preferred, but not required.
• Familiarity with hemodialysis or peritoneal dialysis is a plus.
• Strong interest in nephrology and enthusiasm for developing specialty expertise.
• Spanish-language proficiency is a plus.

Compensation

• Full time, salaried & benefited job but for payroll purposes:$85.95 - $117.50 per Hour
• Compensation is based on experience and qualifications, consistent with applicable requirements.
