Job description

Are you passionate about delivering high-quality nephrology care in a close-knit community? Sierra Nevada Specialty Care is hiring an Advanced Practice Provider (APRN or PA-C) to join our dedicated team. This role offers the unique opportunity to split your time between dialysis rounding and CKD clinic visits, influence patient education programs, and help shape kidney care in rural Nevada.

What You’ll Do

You’ll be an integral part of patient care, collaborating with experienced physicians and dialysis staff while working autonomously to manage your patient panel.

Clinical Care & Patient Management (90%)

  • Provide direct care including assessments, treatment planning, and patient/caregiver education.
  • Conduct monthly dialysis patient visits and collaborate with Medical Directors on treatment plans.
  • Manage CKD clinic patients, providing education on dialysis modalities, transplant options, and conservative care.
  • Lead advanced care planning and post-hospital transitions to reduce re-hospitalizations.
  • Perform hospital rounding, coordinate interdisciplinary care, and ensure effective documentation.

Collaboration & Leadership (10%)

  • Serve as a clinical resource for dialysis staff and hospital teams.
  • Participate in protocol reviews and quality improvement initiatives.
  • Maintain professional standards and foster a respectful, collegial work environment.
What We’re Looking For
  • License: APRN or PA-C with an unrestricted Nevada license.
  • Experience: Minimum 3 years of healthcare experience; nephrology experience preferred but new graduates with 5 years clinical experience considered.
  • Education: Master’s degree from an accredited institution; BLS certification; valid Nevada driver’s license.
  • Skills: Independent, organized, strong communicator, able to multitask and navigate complex patient care scenarios.
  • Flexibility: Willingness to travel between Elko and Carson City.
Physical Requirements
  • Frequent walking, standing, bending, and travel between worksites during the day.
