Job summary

Puzzle Healthcare is seeking a Nurse Practitioner or Physician Assistant to serve one or more skilled nursing facilities in Sapulpa, OK on an onsite, part-time basis.

You’ll round on facilities on weekday daytime schedules, conduct focused assessments, manage pain, wounds and functional decline, and collaborate with physicians, therapists and staff. Documentation is via Puzzle’s EHR, often remotely from home, with RVU-based pay.

• Collaborate with physicians, therapists, and facility staff on individualized recovery plans

• Monitor readmission risk and help close gaps in post-acute care
• Maintain compliance with post-acute and PM&R-related regulations

As an Nurse Practitioner/Physician Assistantin Sapulpa, OK, you’ll provide focused care in nursing facilities helping reduce readmissions and improve recovery while enjoying autonomy, a predictable schedule, and strong team support.

This is an onsite position serving one or more skilled nursing facilities (SNFs) in the Sapulpa, OK area.

Flexible Schedule: Part-Time/RVU-Based, 1–2 days/week; no call, no weekends

What You’ll Do

What You’ll Gain

• Schedule: Weekday daytime hours only; no nights, weekends, or call

• Compensation: This position is compensated through an RVU-based productivity model. In accordance with pay transparency guidelines, the estimated hourly equivalent for this role is $60–$75 per hour, based on experience, productivity, and clinical performance.This hourly equivalent is provided for transparency purposes only and does not represent a guaranteed hourly wage.
