Puzzle Healthcare in Mt. Morris, IL is seeking a Nurse Practitioner or Physician Assistant to provide focused care in SNFs on a weekday daytime schedule.

This onsite role offers autonomy, strong team support, and the chance to help reduce readmissions and support recovery. You'll round in facilities, collaborate with physicians and therapists, document in EHR, monitor readmission risks, and ensure compliance with post-acute regulations.

• Use Puzzle’s EHR to complete documentation (often remotely from home)
• Monitor readmission risk and help close gaps in post-acute care
• Maintain compliance with post-acute and PM&R-related regulations

Puzzle Healthcare is a mission-driven, national post-acute care organization focused on protecting patients during their most vulnerable moments—right after a hospital stay. As a Nurse Practitioner/Physician Assistant in Mt. Morris, IL you’ll provide focused care in nursing facilities helping reduce readmissions and improve recovery while enjoying autonomy, a predictable schedule, and strong team support.

This is an onsite position serving one or more skilled nursing facilities (SNFs) in the Mt. Morris, Illinois area.

Flexible Schedule: Full-Time, Monday-Thursday; no call, no weekends

What You’ll Do

What You’ll Gain

• Schedule: Weekday daytime hours only; no nights, weekends, or call

• Compensation: The anticipated total compensation range for this role is $120,000–$180,000 annually, inclusive of base salary and potential incentive and/or bonus opportunities. Final compensation will be based on experience, location, qualifications, and other lawful considerations.
