Job summary

PartnerCare is seeking an experienced Nurse Practitioner for a part-time weekend role in Jackson, TN. You will provide patient-centered pain management within a collaborative care team, with strong focus on safety and quality outcomes.

Responsibilities include physical exams, orders and interpretation of labs, EHR documentation, and patient education on medications and health promotion. ACLS/BLS and TN licensure are required.

Qualifications

• Completion of an accredited NP program (AANP Certification) is required.
• Current, unrestricted TN NP license with DEA registration.
• CPR, ACLS, and BLS certification are required.
• Experience preferred, but new graduates are welcome to apply.

Responsibilities

• Assess the physical and mental condition of patients by performing comprehensive examinations and obtaining histories.
• Order, collect, interpret, and evaluate labs and other tests to guide care.
• Maintain accurate, timely EHR documentation of treatment plans and outcomes.
• Counsel patients and families about medications, regimens, nutrition, and health promotion.
• Collaborate with physicians and other providers to create safe, balanced pain management plans.

You will work as part of the tight knit care team with the Physicians and other Advanced Practice Providers to ensure the patient has the best possible experience and outcomes.Collaborate with the Physician and other Providers to create a safe, balanced, and patient focused approach to pain management and treatment planAssesses the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories.Orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs.Maintains (accurately, timely and complete) appropriate Electronic Health records detailing the patient’s treatment plans and outcomes. Initiates timely consultation and referral when the problem exceeds scope of practice and/or expertise.Counsels patients and families regarding medical processes, management of the health illness/condition, medication, nutrition, and health promotion. Counsels patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over the counter medications and herbal remedies.Review patient documents to measure the effectiveness and quality of patient care.Our practice focuses on taking care of the patient with a comprehensive plan, delivering high customer service, keeping patients safe and well-informed and reducing patients' pain as much as possible.Ideal Qualifications:Completion of an accredited Nurse Practitioner Program (AANP Certification)Maintains current and unrestricted state license with a DEA with Schedule IIExperienced preferred, but new graduates are welcome to applyCPR certification for Health Care ProvidersACLS and BLS certification required.Desire to work as part of a team environmentFits in with the culture and vision of the organization
