Job summary

Mecklenburg County Public Health seeks an Advanced Practice Provider (NP/PA) for the mobile health units. The role covers STI, family planning, breast and cervical cancer control programs, and PrEP/Rapid ART support, with a high degree of autonomy in patient care.

The position is hybrid with variable hours Mon–Sat, home location at 2845 Beatties Ford Rd, Charlotte, NC, and a salary exempt status. Applicants must meet NC licensing requirements and be bilingual in English/Spanish.

Qualifications

• Graduation from an NP or PA program approved by NC Joint Subcommittee of the Board of Nursing and Board of Medical Examiners.
• Maintain Medical Board License as NP or PA and national certification.
• BLS (CPR & AED) certification valid for at least 90 days after hire.

Responsibilities

• Provide assessment, diagnosis, treatment, education, referral and follow-up in STI, family planning, and related programs.
• Work with PrEP, Rapid ART, TB/Refugee and Colposcopy programs with high autonomy.
• Prescribe medications, review labs, document in EMR, bill and code appropriately.
• Coordinate treatment with staff and assist in other nursing/medication duties as needed.
• Deliver services in mobile health units and community settings, including outreach activities.

Position Summary

Meck Health in Action (MHiA) is an innovative program that uses two mobile clinical units and one mobile dental unit to advance our community health initiatives. All three mobile units function as extensions of the Mecklenburg County Public Health department in our community.

This advance practice provider role provides assessment, diagnosis, treatment, education, referral and follow-up for patients seen in the Clinical Services area of the Health Department primarily in STI, Family Planning, Breast and Cervical Cancer Control Programs in addition to providing support for PrEP, Rapid ART, TB/Refugee and Colposcopy Programs. This position, with a high degree of autonomy, functions as a physician extender and provides healthcare services to include preventive and acute care.

Hours: This is a hybrid position with variable working hours Monday – Saturday with potential to flex hours, not to exceed a 40-hour work week.

Essential Functions

• Ensure compliance with agreement addenda requirements and agency policies and procedures for STI, Family Planning, Breast and Cervical Cancer Control Program (BCCCP), Rapid ART, PrEP, Tb/Refugee and/or Colposcopy programs
• Obtain and review patient histories and perform physical exams and assessments
• Formulate care plan and provides patient counseling and education
• Order and review lab-work and diagnostic tests
• Prescribe medication and determine patient follow up
• Document in EMR in a timely fashion.
• Bill and code appropriately
• Coordinate treatment and plan with clinical staff and other physicians, when indicated
• Assists staff with other nursing or medication related duties as needed
• Participate in public health emergency response activities as needed
• Assist clients in navigating healthcare and social service systems by sharing information on available resources
• Support linkage to specialty care, substance use treatment, behavioral health services, and other healthcare resources by providing recommendations and guidance to patient care team members
• Provide resources to assist with connections to housing, food assistance, transportation, behavioral health, substance use treatment, insurance enrollment, and other social support services through collaboration with patient care team members
• Provide clinical guidance and support to staff as appropriate

For Advance Practice Providers assigned to work in community-based settings including Meck Health in Action (MHiA) program providers:

• Deliver services in mobile health units and community-based settings throughout the service area.
• Develop relationships with community organizations, healthcare providers, and public health partners.
• Participate in community events, health fairs, and targeted outreach initiatives
• Collaborate with outreach teams to engage underserved and vulnerable populations
• Administer vaccinations and support immunization activities, including seasonal, routine, and outbreak-response vaccination programs
• Support harm reduction education and interventions for individuals who use drugs
• Provide basic wound care

Licenses and Certifications

• Maintain Medical Board License as Nurse Practitioner or Physician Assistant.
• Obtain and maintain national certification as Nurse Practitioner or Physician Assistant.
• Certification in Basic Life Support (BLS) for Healthcare Providers (CPR & AED) by the American Heart Association that is valid for at least 90 days after hire.
• Obtain required FEMA certifications (upon hire if not already obtained).

• 2 years’ Advanced Practice Provider experience
• Bilingual (English/Spanish)

• Working knowledge of CDC STI Treatment Guidelines, 2021
• IUD and Hormonal Implant training and experience (for Family Planning providers)
• Tb/LTBI experience (for Tb/Refugee providers)
