Job summary

Endeavor Health seeks an Advanced Practice Provider (APN/PA) to join our Pulmonology team in outpatient settings across Evanston and surrounding sites. You will see new and established patients and hospital follow-ups, with a focus on primary care, hospital medicine, or pulmonology experience. New grads considered for motivated candidates.

The role is full-time (40 hours/week), weekdays 8:30 a.m.–5:00 p.m., with no weekends and no on-call. Sign-on bonus eligible for experienced candidates.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA experience preferred

Responsibilities

• Provide care to patients under the direction of collaborating physicians in the Pulmonology Department.
• Obtain detailed histories, perform examinations, order imaging, labs, and diagnostic tests, and interpret results.
• Instruct patients on prescribed therapies and pre/post operative instructions; maintain records and progress notes.
• Follow patients through their course of treatment in both clinic and hospital settings; collaborate with physicians and health care team.
• Prescribe pharmacologic and non-pharmacologic therapies as appropriate and promote wellness and prevention.
• Participate in development and evaluation of standards of practice; support service line goals and organizational policies.

Experience in primary care, hospital medicine, or pulmonology is desired, but the team is willing to consider motivated new grads.

• Hours: Monday - Friday between 8:30 a.m. and 5:00 p.m.

Job Description

Exciting opportunity to join Endeavor Health in the Pulmonology Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed histories and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes.Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, formulate differential diagnoses, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevention of illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as a resource and consultant. Positively contributes to safety outcomes and promotes a high-quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.
