TapestryHealth is seeking a full-time, hybrid Tapestry Management Provider to deliver in-person clinical support in Kansas City and Johnson County. This role involves proactive patient care through Chronic Care Management and close collaboration with healthcare professionals.

The ideal candidate will travel to assigned facilities about three days a week while working remotely for the remaining two. Competitive compensation ranges from $130K to $135K, including benefits like 401k and PTO.

Qualifications

• Experience in chronic care management, remote patient monitoring, or behavioral health integration preferred.

Responsibilities

• Conduct monthly in-person visits to assigned healthcare facilities.
• Manage between 1 - 10 Tapestry affiliated facilities.
• Deliver and review patient CCM, RPM, Daily Risk reports and BHI reports with facility staff.
• Perform Initial Preventive Visits (IPV) for new patients and re-admits as required for service enrollment.
• Facilitate discussions with medical professionals to align data insights with patient care.
• Ensure clinical observations support report accuracy and provide patient health insights.
• Collaborate with facility staff to enhance proactive care planning and improve outcomes.

Job Description: Tapestry Management Providers (TMPs) play a crucial role in delivering comprehensive, in-person clinical support to healthcare facilities. They ensure proactive patient care through the review and discussion of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) & daily risk reports with facility healthcare professionals, including physicians, nurse practitioners, and social workers.

This is a full-time, hybrid position that combines remote work with weekly travel to assigned skilled nursing facilities in the Kansas City, MO and Johnson County, KS, including Overland Park, Leawood, Prairie Village, and Mission area. Providers are expected to travel within the assigned territory approximately three days with the remaining two days worked remotely.

Key Responsibilities:

• Conduct monthly in-person visits to assigned healthcare facilities.

Salary: The anticipated annual salary for this role is $130K - $135K with benefits including 401k, PTO, and CME. We also reimburse for mileage since travel is involved. This hybrid position follows a location-based compensation structure. The posted salary range represents the potential pay range. Actual compensation will be determined based on the candidate’s primary work location, experience, qualifications, and internal equity considerations, in accordance with applicable pay transparency laws
