Affinity Home Health Care Inc. invites an Advanced Practice Provider (NP or PA) to join a collaborative primary care practice in a well‑established family medicine setting.

The role includes managing 16–18 patients daily with a team of physicians and support staff, using Epic EHR and AI tools for documentation, while upholding patient safety and confidentiality. Monday–Friday, 7am–5:30pm, with comprehensive benefits offered.

Qualifications

• Active NP or PA license with a minimum master’s degree.
• Preferred experience in family medicine; exceptional new graduates considered.
• Strong communication, teamwork, and clinical skills.
