Endeavor Health in Naperville is seeking an Advanced Practice Provider to join the Endocrinology Department for outpatient care. You will work under collaborating physicians, taking detailed histories, performing exams, ordering tests, and interpreting results in a team-based setting.
Responsibilities include prescribing therapies, teaching patients, and contributing to the development and evaluation of practice standards to improve patient outcomes.
Hourly Pay Range: $60.18 - $75.98 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
Exciting opportunity to join Endeavor Health in the Endocrinology Department. Under the direction of the collaborating physician(s), we provide care to patients. We obtain detailed history and perform physical examinations, order imaging, lab tests, and diagnostic procedures and interpret results. We instruct patients on the prescribed therapeutic regimen and pre/post-operative instructions.
We maintain records and write progress notes on status, treatments and procedures. We follow patients through their course of treatment in the clinic and hospital setting. We collaborate with physicians and the health care team to achieve optimal patient outcomes. We prescribe pharmacologic and non-pharmacologic therapies as appropriate. We demonstrate evidence-based practice and participate in the development, implementation, and evaluation of standards of practice in the specialty. We assess the learning needs of patients and families across the continuum, and employ strategies, interventions and teaching to promote, maintain, and improve health.
We provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in direct management of acute and chronic illness and disease. We promote wellness and prevent illness and injury in various settings for individuals, families, and communities. We work autonomously and in interdisciplinary teams as resources and consultants. We positively contribute to safety outcomes and promote high-quality patient experience. We participate in the development, implementation, and evaluation of service line goals and support the strategic plan of the organization. We adhere to all Endeavor Health policies and procedures.
