HealthPartners Digestive Care is seeking an Advanced Practice Provider to join a team of GI physicians and APPs. The role manages a full patient caseload, performs new patient assessments, chronic disease management, and collaborative care within a fast-paced GI setting.

GI Advanced Practice Providers manage a fullpatient caseload and are trusted experts in this specialized field.They provide new patient assessment for patients with liver andgeneral GI disease; ongoing chronic disease management for thispatient population; and between-visit partnership with patients inthe management of their disease. These providers are an integral partof the GI team and the patient care they provide is foundational andessential in caring for the HealthPartners patient population with GIand liver disease.
