HealthPartners in Minnesota seeks an Advanced Practice Provider (NP/PA) to join Digestive Care, managing a full patient caseload in clinic and hospital settings alongside GI physicians.

You will perform new patient assessments, chronic disease management, and collaborate across the GI team to deliver high quality care. Requires MN NP/PA licensure, BLS, and DEA certification, with strong communication skills.

Qualifications

  • Successful completion of an approved Nurse Practitioner program or accredited Physician Assistant program.
  • Current MN NP/PA licensure with RN license
  • Current APP/PA certification
  • Minnesota prescriptive privileges approval
  • BLS and DEA certification are required

Responsibilities

  • Obtain history and perform examinations.
  • Order labs and coordinate follow-up care.
  • Educate patients and families about treatment and follow-up.
  • Communicate with patients between visits and follow up on results.
  • Collaborate with other health professionals and coordinate care.
  • Complete all required paperwork timely and accurately.
  • Coordinate inpatient service flow and consultations.
  • Discuss and develop diagnosis and treatment plans with gastroenterology staff.
  • Prescribe medications as indicated and document in EMR.

POSITIONSUMMARY STATEMENT

RegionsDigestive Care is staffed by Advanced Practice Providers who arededicated to clinic and hospital service, as well as GI physicianswho have varying clinic rotations along with their procedural, andhospital schedules. GI Advanced Practice Providers manage a fullpatient caseload and are trusted experts in this specialized field.They provide new patient assessment for patients with liver andgeneral GI disease; ongoing chronic disease management for thispatient population; and between-visit partnership with patients inthe management of their disease. These providers are an integral partof the GI team and the patient care they provide is foundational andessential in caring for the HealthPartners patient population with GIand liver disease. Performs other duties as assigned.

MAJORJOB DUTIES AND TASKS

PHYSICALREQUIREMENTS

Standsand walks up to 90% of shift while delivering patient care andtreatment activities. Bends, reaches, twists and stoops frequentlywhile performing these same activities. Uses hands and fingersdaily while using equipment, performing treatments and keyboarding.Pushing and/or pulling may be required when transporting patients andequipment, performing CPR, or when intervening with patients. Lowlevel work can occur when performing CPR or wheelchair footrestadjustments.
