Windom Area Health is seeking an Advanced Practice Provider to deliver high-quality Primary Care and Urgent Care services to patients of all ages in our Windom/ Fairmont region. You will diagnose, treat, and manage a broad range of acute and chronic conditions with an emphasis on preventive care and accessible services.

Ideal candidates will have PA or CNP credentials and Minnesota licensure, with DEA eligibility and BLS; ACLS/PALS preferred.

Qualifications

  • Graduate of an accredited Physician Assistant (PA) or Certified Nurse Practitioner (CNP) program.
  • Minimum of one (1) year of Primary Care, Family Practice, Internal Medicine, Emergency Medicine, or Urgent Care experience preferred.
  • Physician Assistant (PA) or Certified Nurse Practitioner (CNP), licensed in Minnesota; able to obtain DEA certification in Minnesota; meet credentialing and privileging criteria; BLS certification; ACLS/PALS preferred.

Responsibilities

  • Provide comprehensive Primary Care and Urgent Care services for pediatric and adult patients.
  • Evaluate, diagnose, and treat acute illnesses, injuries, and chronic medical conditions.
  • Deliver preventive care, wellness screenings, and health education initiatives.
  • Conduct comprehensive patient histories, physical examinations, and targeted assessments.
  • Order and interpret diagnostic testing and develop individualized treatment plans.
  • Prescribe medications and therapies per clinical guidelines and scope of practice.
  • Perform office-based procedures and interventions.
  • Coordinate care with physicians, nursing staff, specialists, and community resources.
  • Advocate for patients and connect them with services to improve long-term health outcomes.
  • Foster continuity of care through ongoing patient engagement.

Located in Fairmont, MN, our clinic is conveniently accessible from Blue Earth, Jackson, Martin County, Mankato, and northern Iowa communities.

Requirements

Graduate of an accredited Physician Assistant (PA) or Certified Nurse Practitioner (CNP) program. Minimum of one (1) year of Primary Care, Family Practice, Internal Medicine, Emergency Medicine, or Urgent Care experience preferred.

Required credentials:

Physician Assistant (PA) or Certified Nurse Practitioner (CNP), licensed in the state of Minnesota. Ability to obtain and maintain DEA certification for the State of Minnesota. Meet threshold criteria/qualifications for Credentialing and Privileging. BLS Certified. ACLS and/or PALS certification preferred.

This position would work 3 ten-hour shifts per week, plus every other weekend (4 hours each day).
