Job summary

Titan Placement Group is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for an outpatient primary care role in the Greater Sacramento area. You will provide care across all ages, manage acute and chronic conditions, and emphasize preventive services in a supportive, team-based setting.

Expect a Monday–Friday schedule with ~3 patients per hour, telehealth visits, and a comprehensive benefits package.

Qualifications

• Active California NP or PA license required.
• Current DEA and BLS certifications.
• 1–2 years outpatient primary care experience preferred.
• Community health or FQHC experience is a plus.
• Bilingual candidates encouraged: Spanish, Russian, Farsi, Pashto.

Responsibilities

• Provide comprehensive outpatient primary care for patients of all ages.
• Manage acute and chronic disease with preventive care and wellness visits.
• Collaborate with physicians and clinical staff in a team-based environment.

We’re hiring an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) who wants to make a meaningful difference while building long-term relationships with patients in a collaborative outpatient practice.

You’ll step into an outpatient role providing comprehensive primary care for patients of all ages, including acute and chronic disease management, preventive care, and wellness visits. You’ll have the autonomy to practice at the top of your license while collaborating with experienced physicians and clinical staff in a supportive, team-based environment serving diverse communities throughout the Greater Sacramento area.

Compensation & Benefits

• Base salary: $145,600-$215,000 annually
