Job summary

Acclaim Health seeks an Advance Practice Provider for the 4th Trimester Clinic at John Peter Smith Hospital in Fort Worth, TX. The role involves postpartum care, blood pressure management, depression screening, contraception, lactation support, and resource referrals.

Qualifications

• 1-2 years’ experience as an Advanced Practice Provider in Obstetrics and Gynecology.
• Master’s degree in nursing from CCNE/NLNAC-accredited program, or NLNAC.
• Certification and Texas APRN/PA licensure required (NCC, ACNM, or NCCPA).

Responsibilities

• Collaborates with physician to oversee and coordinate multidisciplinary care.
• Reviews patient charts to assist in diagnosis, treatment, and care management.
• Make appropriate patient referrals as necessary.
• Ensure services comply with JPS Health Network and Acclaim policies.
