Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long‑term care members in a nursing home setting. You will deliver collaborative primary care, develop individualized care plans, and participate in facility‑level care team meetings with families and staff.
You will coordinate with PCPs, manage prior authorizations, and support quality initiatives while maintaining high‑quality, member‑centered care across transitions.

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role, you'll be responsible for a panel of enrolled members receiving nursing home care. You'll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members. As part of your role, you'll conduct comprehensive assessments and develop individualized care plans for each member. You'll actively participate in nursing facility care team meetings, maintain frequent communication with members' families and loved ones, and ensure the care team stays aligned with members' goals and needs.
