Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care members and provide comprehensive primary care in nursing facilities.

You will work with a clinical leadership team to develop individualized care plans, participate in care team meetings, and communicate with families and PCPs to ensure alignment with member goals and needs.

Qualifications

  • 6+ months experience as a Nurse Practitioner or Physician Assistant in long-term care.
  • Master of Science in Nursing as an NP or a Master’s degree as PA from an accredited program.
  • Experience in nursing home settings or with long-term resident care preferred.
  • Ability to engage physicians, members, and families effectively.

Responsibilities

  • Provides collaborative primary care to a panel of long-term care members.
  • Leads ISNP activities and advanced care planning to reduce hospitalizations.
  • Manages prior authorizations, Part A/B services, and medication reviews.
  • Communicates with families, PCPs, and care teams about goals of care.
  • Contributes to quality initiatives and care coordination efforts.
  • Maintains licensure, regulatory compliance, and documentation accuracy.

About the Role

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. In this role, you\'ll be responsible for a panel of enrolled members receiving nursing home care. You\'ll be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members. As part of your role, you\'ll conduct comprehensive assessments and develop individualized care plans for each member. You\'ll actively participate in nursing facility care team meetings, maintain frequent communication with members\' families and loved ones, and ensure the care team stays aligned with members\' goals and needs.

Key Tasks and Responsibilities
  • Provides collaborative primary care and services to a panel of members
  • Delivers ISNP Model of Care activities, develops and implements advanced care planning, and embraces a treat-in-plan model to avoid unnecessary hospitalizations
  • Facilitates and administers Health Plan clinical operations functions—including prior authorizations, Part A and B services, medication reviews, and quality gap closure (HEDIS and STARs); supports membership growth
  • Serves as a trusted advisor to members, loved ones, nursing home staff, and colleagues
  • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and interdisciplinary care team members on care needs, goals of care, and treatment plans/changes
  • Supports overall market goals/targets and balances member caseloads
  • Promotes Longevity Health with members, loved ones, nursing facilities and other stakeholders
  • Functions autonomously and is accountable for practice
  • Establishes meaningful relationships and operates effectively in a nursing home environment
  • Is member-centric and customer service oriented, ensuring high-quality care for Longevity Health members
  • Performs other related duties as assigned
Requirements
Knowledge, Experience and Education
  • 6+ months experience as a Nurse Practitioner or Physician Assistant
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program
  • Experience working in the nursing home environment is strongly desired
  • Computer/technology proficiency required
  • The ability to engage and communicate effectively with physicians, members, and family members
  • Self-directed but functions well in a team environment
  • Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
  • Demonstrated critical thinking, organization, and prioritization skills
Credentials & Coverage
  • State unrestricted licensed Advanced Practice Nurse with National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriactric or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant (NCCPA board certified) required
  • DEA License required
  • Must have an active national provider number (NPI) and an active Medicare provider number
  • Must be eligible to participate in the Medicare program as a provider and registered
  • Valid state driver\'s license with a good driving record and proof of automobile insurance required
  • Auto liability insurance declaration page with minimum limits required by your state of residence; provide proof of coverage annually
