Qualifications

The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

Additional Job Description

POSITION SUMMARY

The Advanced Practice Professional (APP)has coordination and clinical skills required for the comprehensivemanagement of vascular surgery patients ( including biological,social, and psychological assessments). The APP coordinatesinvolvement of medical specialty teams necessary to develop acomprehensive plan for the patient and reviews the plan of carewith the other members of the clinical team, participates indirecting that care, performs diagnostic/therapeutic procedures andcoordinates discharge planning (as applicable). The APPparticipates in the education of residents and medical/nursingstudents.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable . Such aservice may include but is not limited to.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER

EDUCATION, CERTIFICATION, AND/OR LICENSURE

1. Graduate of a CCNE accredited nursing program with Master’s Degreein Nursing .
2. National Certification by one of the recognized APRN CertificationCenters .
3. Licensure or eligibility for licensure as a professional RegisteredNurse and Advanced Practice Registered Nurse in WestVirginia.
4. Department Specific : APRNs who will be privileged as First Assistsin the OR (as part of their role) are required to complete RNFA certification.
5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion ofthe department.

EXPERIENCE

1. Vascular surgery or a cute care experience preferred.
2. Eligible new graduates will be considered.

PREFERRED QUALIFICATIONS

EDUCATION, CERTIFICATION, AND/OR LICENSURE

1. ACNPs preferred but will consider FNPs.

1. Bachelor’s Degree or Master’s Degree as a Physician Assistantrequired.
2. Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required.
3. Certification by the National Commission for Certification ofPhysician Assistants required.
4. Must be licensed or eligible for licensure by the WV Board ofMedicine or WV Board of Osteopathic Medicine .
5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion ofthe department.

EXPERIENCE

1. Vascular surgery or a cute car e experience preferred.

CORE DUTIES AND RESPONSIBILITIES

1. Manages comprehensive and individualized care for vascular surgerypatients and their families in the inpatient phase ofhospitalization immediate post discharge and in the ambulatorysetting.
2. Completes daily rounds; consults and/orpre-procedure/evaluations.
3. Completes integrated assessments.
4. Participates in the perioperative management of vascular surgerypatients which may include but is not limited to first assist inthe operative suite.
5. With the collaboration of vascular surgery faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate.
6. Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
7. Consults and communicates with clinic and hospital staff regardingabnormal findings in the evaluations of patients.
8. Performs succinct comprehensive clinical evaluations and documentsfindings in medical record.
9. Facilitates patient discharge by coordinating appropriate servicesand tests prior to discharge necessary for outpatient follow-upappointments in the clinic (as applicable).
10. Develops a comprehensive individualized plan of care for vascularsurgery patients, which includes teaching, discharge planning andnecessary referral to other members of the health care team asnecessary.
11. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
12. Identifies unstable or complex vascular surgery patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting.
13. Recognizes emergency situations and initiates effective emergencycare.
14. Evaluates vascular surgery patients and serves as a coordinator forfollow up procedures and tests in the ambulatory setting (asapplicable).
15. Assists in the collection of outcome measures (asapplicable).
16. Demonstrates critical thinking and diagnostic reasoning skills inclinical decision making by the identification, evaluation, andmanagement of the individual needs of patients/families.
17. Demonstrates the ability to utilize equipment in a safemanner.
18. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
19. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families, and the community.
20. Promotes an environment that facilitates learning by respondingtoappropriate patient/family preferences, priorities, and readinessto learn.
21. Updates own knowledge base throughout the year.
22. Completes all identified clinical competencies.
23. Serves as a resource person for staff caring for the vascularsurgery patient.
24. Provides educational programs to professional and paraprofessionalpersonnel in area of expertise.
25. Participates in community programs, distribution of literature, asappropriate in area of expertise.
26. Documents the plan of care, interventions, and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
27. Collaborates with other leaders and staff to effectively achievepatient outcome-based improvements in care.
28. Provides leadership in the development and implantation of changesin the vascular surgery practice that positively impact patientoutcomes.
29. Works collaboratively with medical and nursing staff to facilitatecreative problem solving and provide professional support.
30. Participates and initiates inter-disciplinary approaches to patientcare, program development and education. Serves as a professionalrole model.
31. Promotes and practices innovation of the expanded role in thedelivery of care to vascular surgery patients and familiesthroughout the inpatient hospitalization, and the ambulatorysetting.
32. Participates and provides leadership for committees and taskforces.
33. Documents hours and submits billing reports, as appropriate.
34. Reviews and interprets vascular surgery data from sources such aspatient registries, clinical and financial reports, length of stay,access to care, and cost per case reports and recommends/implementschanges as necessary.
35. Participates in clinical trials and research studies with thePrincipal Investigator.
36. Assists vascular surgery Faculty, Administrators and Directors inthe collection of outcome measures.
37. Other duties/projects as assigned.
