UHA University Health Associates seeks an Advanced Practice Professional to coordinate comprehensive vascular surgery care. The APP will work within a multidisciplinary team, perform evaluations, guide perioperative management, and coordinate discharge planning for vascular patients in inpatient and ambulatory settings.

Education and certification requirements include a CCNE-accredited MSN, national APRN certification, and WV licensure eligibility, with RNFA and ACLS/BLS preferred.

Qualifications

  • Master’s Degree in Nursing from a CCNE-accredited program.
  • National Certification by recognized APRN Certification Centers.
  • Licensure or eligibility for licensure as RN/APRN in West Virginia.
  • RNFA certification for First Assist in the OR preferred.
  • AHA BLS with Advanced Life Support training per department.

Responsabilités

  • Manages comprehensive and individualized care for vascular surgery patients and families in inpatient, post-discharge and ambulatory settings.
  • Completes daily rounds; consults and/or pre-procedure evaluations.
  • Completes integrated assessments and participates in perioperative management.

The Advanced Practice Professional (APP) provides high-quality,patient-centered care as a member of the interdisciplinaryhealthcare team. Working collaboratively with physicians, nurses,and other healthcare professionals, the APP assesses, diagnoses,treats, and manages patients within their scope of practice andarea of specialty. The APP is responsible for deliveringevidence-based care, promoting positive patient outcomes,maintaining professional and regulatory standards, and supportingthe clinical, educational, and operational goals of the departmentand organization.

Additional Job Description

POSITION SUMMARY : The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensivemanagement of vascular surgery patients (including biological,social, and psychological assessments). The APP coordinatesinvolvement of medical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of carewith the other members of the clinical team, participates indirecting that care, performs diagnostic/therapeutic procedures andcoordinates discharge planning (as applicable). The APP participates in the education of residents and medical/nursingstudents.

Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable. Such aservice may include but is not limited to.
MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  1. 1. Graduate of a CCNE accredited nursing program with Master’sDegree in Nursing.
  2. 2. National Certification by one of the recognized APRNCertification Centers.
  3. 3. Licensure or eligibility for licensure as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia.
  4. 4. Department Specific : APRNs who will be privileged as FirstAssists in the OR (as part of their role) are required tocomplete RNFA certification.
  5. 5. American Heart Association BLS Certification required withadditional Advanced Life Support training at the discretion of the department.
EXPERIENCE:
  1. 1. Vascular surgery or acute care experience preferred.
  2. 2. Eligible new graduates will be considered.
PREFERRED QUALIFICATIONS :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  1. 1. ACNPs preferred but will consider FNPs.
MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT :
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
  1. 1. Bachelor’s Degree or Master’s Degree as a Physician Assistant required.
  2. 2. Graduate of an ARC-PA approved Physician Assistant (PA) training program required.
  3. 3. Certification by the National Commission for Certification of Physician Assistants required.
  4. 4. Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.
  5. 5. American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
EXPERIENCE:
  1. 1. Vascular surgery or acute care experience preferred.

CORE DUTIES AND RESPONSIBILITIES:

The statements describedhere are intended to describe the general nature of work being performed by people assigned to this position. They are notintended to be constructed as an all-inclusive list of allresponsibilities and duties. Other duties may be assigned.

  1. 1. Manages comprehensive and individualized care for vascularsurgery patients and their families in the inpatient phase ofhospitalization immediate post discharge and in the ambulatorysetting.
  2. 2. Completes daily rounds; consults and/orpre-procedure/evaluations.
  3. 3. Completes integrated assessments.
  4. 4. Participates in the perioperative management of vascular surgerypatients which may include but is not limited to first assist inthe operative suite.
  5. 5. With the collaboration of vascular surgery faculty or byfollowing established clinical Guidelines and Protocols, ordersadditional tests or consults as deemed appropriate.
  6. 6. Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.
  7. 7. Consults and communicates with clinic and hospital staffregarding abnormal findings in the evaluations of patients.
  8. 8. Performs succinct comprehensive clinical evaluations anddocuments findings in medical record.
  9. 9. Facilitates patient discharge by coordinating appropriateservices and tests prior to discharge necessary for outpatientfollow-up appointments in the clinic (as applicable).
  10. 10. Develops a comprehensive individualized plan of care forvascular surgery patients, which includes teaching, dischargeplanning and necessary referral to other members of the health careteam as necessary.
  11. 11. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.
  12. 12. Identifies unstable or complex vascular surgery patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting.
  13. 13. Recognizes emergency situations and initiates effectiveemergency care.
  14. 14. Evaluates vascular surgery patients and serves as a coordinatorfor follow up procedures and tests in the ambulatory setting (asapplicable).
  15. 15. Assists in the collection of outcome measures (asapplicable).
  16. 16. Demonstrates critical thinking and diagnostic reasoning skillsin clinical decision making by the identification, evaluation, andmanagement of the individual needs of patients/families.
  17. 17. Demonstrates the ability to utilize equipment in a safemanner.
  18. 18. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.
  19. 19. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families, and the community.
  20. 20. Promotes an environment that facilitates learning by respondingto appropriate patient/family preferences, priorities, andreadiness to learn.
  21. 21. Updates own knowledge base throughout the year.
  22. 22. Completes all identified clinical competencies.
  23. 23. Serves as a resource person for staff caring for the vascularsurgery patient.
  24. 24. Provides educational programs to professional andparaprofessional personnel in area of expertise.
  25. 25. Participates in community programs, distribution of literature,as appropriate in area of expertise.
  26. 26. Documents the plan of care, interventions, and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation.
  27. 27. Collaborates with other leaders and staff to effectivelyachieve patient outcome-based improvements in care.
  28. 28. Provides leadership in the development and implantation ofchanges in the vascular surgery practice that positively impactpatient outcomes.
  29. 29. Works collaboratively with medical and nursing staff tofacilitate creative problem solving and provide professionalsupport.
  30. 30. Participates and initiates inter-disciplinary approaches topatient care, program development and education. Serves as aprofessional role model.
  31. 31. Promotes and practices innovation of the expanded role in thedelivery of care to vascular surgery patients and familiesthroughout the inpatient hospitalization, and the ambulatorysetting.
  32. 32. Participates and provides leadership for committees and taskforces.
  33. 33. Documents hours and submits billing reports, asappropriate.
  34. 34. Reviews and interprets vascular surgery data from sources suchas patient registries, clinical and financial reports, length ofstay, access to care, and cost per case reports andrecommends/implements changes as necessary.
  35. 35. Participates in clinical trials and research studies with thePrincipal Investigator.
  36. 36. Assists vascular surgery Faculty, Administrators and Directorsin the collection of outcome measures.
  37. 37. Other duties/projects as assigned.
