Job summary

The University of Michigan's Michigan Medicine is seeking an Advanced Practice Professional (NP or PA) to join the Partners for Children-Little Heroes League program at C.S. Mott Children’s Hospital.

The APP will perform home-based assessments, symptom management, and care coordination, joining specialty clinic appointments and virtual visits as needed. Qualified NPs must hold Michigan licenses and Pediatric NP certification, with 3 years of relevant experience; PAs require Michigan PA license,

Qualifications

• Current licensure to practice as an RN and NP in Michigan, and NP/Ped exams/certifications.
• PA route requires Michigan PA license, ARC-PA program, and NCCPA certification.
• 3 years of direct care for medically complex children or palliative services within 5 years.
• DEA registration and BLS certification are required.
• Credentialing and privileging must be completed successfully.

Responsibilities

• Coordinate care for chronic, complex pediatric patients up to age 25 under physician supervision.
• Perform at-home, telehealth, clinic, or in-patient assessments every 2 weeks to 2 months and update records.
• Create and update care plans under physician supervision and conduct yearly comprehensive assessments.
• Attend patient/family/provider/school meetings as an advocate for the patient.
• Order tests and prescribe medications; coordinate with subspecialty care providers.

Job Summary

The C.S. Mott Childrens Hospital is hiring an Advanced Practice Professional to become part of the Partners for Children-Little Heroes League program.

The Partners for Children-Little Heroes League program provides home, hospital/clinic, and community-based services for children with chronic complex medical conditions who receive specialty and hospital-based care at C.S. Mott Childrens Hospital. The APP will perform assessments, symptom management, and care management visits in the patient's home, join specialty clinic appointments, and complete virtual visits.

Responsibilities*

Hospital and Community program Partners for Children-Little Heroes League.

• Under physician supervision, coordinates care of chronic, complex infants, children and adolescents to children age 25
• Specifically, role requires the APP to perform patient assessments at intervals of every two (2) weeks to two (2) months via at-home, telehealth, clinic, or in-patient visits, to gather information obtained from specialty visits, primary care visits, and hospital encounters and to create and update records that consolidate case summaries, problem lists, and medication lists
• Create and update care plans, under physician supervision.
• Conduct and document yearly comprehensive assessments on patients
• Participate in patient/family/provider/school meetings as an advocate for the patient when needed
• Assess and manage acute symptoms/problems either with direct care or collaborating wtih subspecialty care providers
• Participate in bi-weekly interdisciplinary team meetings, and weekly rounds to discuss new and active patients
• Communicate assessments and care plans to the primary care provider and specialty consultants
• Order tests and prescribe medication
• Confer with attending physicians and communicate with other care providers/consultants to coordinate clinical care
• Maintain accurate documentation regarding patient status, evaluations, and treatment
• Perform other related duties as requested by the attending physician
• Utilize new improvements and/or technologies that relate to job assignment

Required Qualifications*

Required Qualifications NP

• Current licensure to practice as a Registered Nurse and Nurse Practitioner in the State of Michigan
• National Certification as a Pediatric Acute Care Nurse Practitioner or Pediatric Primary Care Nurse Practitioner
• Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
• Graduation from an accredited Nurse Practitioner program.
• 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services within the last 5 years.
• Current DEA registration
• AHA Approved BLS

Required Qualifications PA

• Current license to practice as a Physician Assistant in the State of Michigan.
• Satisfactory completion of a physician assistant program accredited by ARC-PA or one of its predecessor agencies.
• Certification by the National Commission on Certification for Physician Assistants (NCCPA).
• 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services within the last 5 years.
• State of Michigan Controlled Substance License Required
• Current DEA registration
• AHA Approved BLS

Work Schedule
