This is an Advanced Practice Professional (APP) job opening. Both qualified NP and PA applicants will be considered. A total of one (1), 0.5 FTE, Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.

Job Summary

The C.S. Mott Childrens Hospital is hiring a part time Advanced Practice Professional to become part of the community program Partners for Children-Little Heroes League and the hospital-based Stepping Stones Pediatric Palliative Care Consult Team. The Partners for Children-Little Heroes League program provides home and community-based services for children with chronic complex medical conditions who receive specialty and hospital-based care at C.S. Mott Childrens Hospital. The APP will perform assessments and care management visits in the patients home, join specialty clinic appointments, and complete virtual video visits. The hospital-based Stepping Stones Pediatric Palliative Care Program provides palliative care across the spectrum of complex, chronic, and life limiting illnesses. The program focuses on symptom management, decision support, and advanced care planning for inpatients as well as children visiting the Mott Clinics (in person and virtually). This is a combined position, and the APP will be assigned a regular schedule with the Partners for Children and Stepping Stones Consult teams.

Responsibilities

Hospital and Community program Partners for Children-Little Heroes League

• Under physician supervision, coordinates care of chronic, complex infants, children and adolescents to children age 25
• Specifically, role requires the APP to perform patient assessments at intervals of every two (2) weeks to two (2) months via at-home, telehealth, clinic, or in-patient visits, to gather information obtained from specialty visits, primary care visits, and hospital encounters and to create and update records that consolidate case summaries, problem lists, and medication lists
• Create and update care plans, under physician supervision.
• Conduct and document yearly comprehensive assessments on patients
• Participate in patient/family/provider/school meetings as an advocate for the patient when needed
• Participate in bi-weekly interdisciplinary team meetings, and weekly rounds to discuss new and active patients
• Communicate assessments and care plans to the primary care provider and specialty consultants
• Order tests and prescribe medication
• Confer with attending physicians and communicate with other care providers/consultants to coordinate clinical care
• Maintain accurate documentation regarding patient status, evaluations, and treatment
• Perform other related duties as requested by the attending physician
• Utilize new improvements and/or technologies that relate to job assignment

Pediatric Palliative Care Inpatient Consult Team

• Provide comprehensive pediatric palliative care within a collaborative practice model under the supervision of the collaborating physician, in accordance with state regulations and hospital bylaws, across inpatient and ambulatory settings.
• Conduct in-person and virtual initial and follow-up consultations for patients from the perinatal period through young adulthood to assess symptoms, clarify patient and family goals and values, and develop care plans.
• Perform comprehensive assessments, including physical, cognitive, and psychosocial status.
• Manage complex symptoms associated with serious illness using pharmacologic and non-pharmacologic interventions.
• Assess goals of care, including advance care planning and hospice eligibility.
• Order, interpret, and recommend diagnostic tests and prescribe treatments based on clinical assessment.
• Collaborate effectively with interdisciplinary teams and other healthcare providers across care settings.
• Provide education and serve as a resource for patients, families, healthcare professionals, and learners.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in quality improvement initiatives and support the development of departmental education.
• Maintain current clinical knowledge and incorporate evidence-based practices and relevant technologies.
• Engage in ongoing self-assessment, professional development, and maintenance of licensure and credentials, including participation in professional organizations.
• Demonstrate strong communication, organizational, and interpersonal skills.

Required Qualifications

• Current licensure to practice as a Registered Nurse and Nurse Practitioner in the State of Michigan
• National Certification as a Pediatric Acute Care Nurse Practitioner or Pediatric Primary Care Nurse Practitioner
• Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
• Graduation from an accredited Nurse Practitioner program.
• 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services
• Current DEA registration
• AHA Approved BLS

Required Qualifications PA

• Current license to practice as a Physician Assistant in the State of Michigan.
• Satisfactory completion of a physician assistant program accredited by ARC-PA or one of its predecessor agencies.
• Certification by the National Commission on Certification for Physician Assistants (NCCPA).
• 3 years of direct patient care experience caring for medically complex children with advanced illnesses and their families or children receiving palliative services
• State of Michigan Controlled Substance License Required
• Current DEA registration
• AHA Approved BLS

Work Schedule

Hours/Week: 20 Hours/Week Shift/Hours/Days: 0.5 FTE, day shift M-F APPs will take call. Weekend and holiday coverage may be required.
