Job summary

Contentnea Health in eastern North Carolina seeks an experienced clinician to provide outpatient medical care in partnership with supervising physicians. The role involves comprehensive patient assessments, documentation in the EHR, medication management, and patient education within an established community health setting.

Candidates must hold current APRN or PA licensure in NC, DEA license, and BLS certification.

Qualifications

• Current licensure as an Advanced Practice Registered Nurse in good standing with the North Carolina Board of Nursing.
• Current licensure as a Physician Assistant in good standing with the North Carolina Medical Board.
• Current Drug Enforcement Agency (DEA) licensure.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Practices medical care for each patient visit.
• Completes pertinent history and physical exam.
• Documents chief complaint, history of present illness, pertinent review of systems, and physical exam in EHR.
• Reviews and updates past medical history, family history, social history, medications, and allergies in EHR.
• Reviews vital signs and results from prior orders.
• Places new orders and performs procedures as indicated.
• Administers medications and immunizations.
• Determines and documents assessment and plan.
• Prescribes medications and provides patient education, guidance and counseling.
• Arranges follow-up care and adheres to scheduling templates.

Job Summary

Provides outpatient medical care in partnership with one or more supervising physicians.

Responsibilities And Duties

• Practices medical care for each patient visit
  • Completes pertinent history and physical exam.
  • Documents chief complaint, history of present illness, pertinent review of systems, and physical exam in electronic health record (“EHR”)
  • Reviews and updates past medical history, family history, social history, medications, and allergies in EHR
  • Reviews vital signs
  • Reviews results and reports from prior orders
  • Places new orders
  • Performs and documents procedures
  • Administers medications and immunizations
  • Determines and documents assessment and plan
  • Prescribes medications
  • Provides patient education, guidance, and counseling.
  • Arranges plan for follow-up
  • Signs EHR encounter within timelines in established policies and procedures
  • Responds to related patient questions or needs which arise following encounter.
  • Adheres to established scheduling templates in provision of patient care.
• Participates in quality metrics directives and departmental quality improvement (QI) projects
  • Attends practitioners’ meetings regularly.
  • Attends clinical site meetings regularly.
  • Attends practice‑wide meetings regularly.
  • Reads emails and other platforms of communication within organization.
  • Completes peer review.
  • Complies with Clinical Quality Measures expectations.
  • Uses resources to verify standards of care.
  • Modifies workflows as determined by department to attain QI objectives.
  • Consistently documents activities related to QI projects.
  • Participates in on-call patient rotation.
  • Maintains availability to receive phone calls throughout dates of on-call service.
  • Responds to on-call contact within timelines in established policies and procedures.
  • Documents on-call patient care in EHR.
  • Notifies patient's primary care physician of on-call patient care.

Qualifications And Skills
